KSA Mini-Grant Application The purpose of the KSA Member Mini-Grant Program is to expand the reach of KSA by providing funding for KSA members to help them create storytelling related projects. The purpose of the Mini-grant application is to provide grant proposal reviewers with a description of your project that will enable them to understand it and to assess your ability to carry it out. To apply: Submit the following information in the order requested below as your grant application. Where word limits have been given, please do not go over those limits. About You * KSA member applicant name: * KSA member applicant contact information: Email (if applicable) Phone Mailing address: (If member applicant is an organization or family, please give the membership name and identify an individual contact person. If the member applicant is under 18, please give the name of the member and identify the parent or guardian who will sign the application form and grant agreement form if the funding is awarded.) About Your Project * Project title: (what KSA should call this in publicity if your project is funded - no longer than 10 words please) * Description of your project (Maximum 800 words) Include what, when, where, how, and who will benefit. About Project Personnel * Identify Your Project Partner: Name: Contact information (email if applicable, website if applicable), phone, mailing address, contact person if partner is not an individual * What are your project partner’s responsibilities in this project? (300 word maximum, if includes funding, please state that here.) * What are your responsibilities in this project? (300 word maximum, if includes funding, please state that here.) * Describe your project management experience? (500 word maximum; assure grant reviewers that you have experiences that you can draw upon to see this project through) About Project Money (where it will come from & how you will spend it) * Where will your matching money come from? List the source(s) and how much each will provide. If some of your match will be in-kind, be sure to include that. (Remember this grant requires a match. Example: To request $100, a match of $100 is required. At least one half of the match/$50 must be cash. The other half of the match could be in-kind value. If volunteer hours are part of your in-kind value, use $10 per hour as the volunteer hourly value.) * Project Budget (Your project budget will not only help the Grant proposal reviewers see that you have the required funds, but it will also help the reviewers evaluate the feasibility of your project given your available funding and your anticipated expenses.) The letter should express their support for your project, including the specific amount and type (cash or in-kind) of funding they promise to provide. Your Project and KSA * Explain how your project supports the KSA Mission, including the specific KSA Goals supported (Maximum 400 words; KSA Goals may be listed by Goal number. See KSA Mission and Goals) * How will you publicize KSA support of this project? (Social media? Print media? Broadcast media? (optional) * If there is something else KSA proposal reviewers might need to know to assure them this will be a successful project, please include that here (Maximum 500 words) Signature Requirements Your application must be signed and dated by you (parent/guardian if KSA member is under 18) and by your Project Partner. (In lieu of your Project Partner's signature on the application you may submit a letter from your Project Partner. In such a letter, they must include their responsibilities as described in this application and state they agree with them; provide any additional information of their choosing; and sign the letter.) If your application is funded -- * You will receive a Grant Acceptance Letter (via email) to sign and return. Then, 90% of your grant award will be mailed to you. * To receive the final 10% of your award: Within 30 days of the end of your project, you submit a Project Final Report, which must include the following: o In what ways was your project successful? (What happened? Who benefited? How many people benefited? How did they benefit?) (maximum 600 words)  o How you publicized KSA’s support for your project. (You’ll need evidence. Example: photos, screen shots of websites, flyers, press releases, newsletter articles, etc.)  o How you used the funding from KSA.  How to Submit Your Mini-Grant Application: Your application may be submitted by email to grants@kystory.org or by postal mail to KSA, P O Box 354, Frankfort, KY 40602.
